Ban certain high-deductible or high co-pay health insurance policies

To prohibit “third party administrator” health insurance coverages that require a service copay of more than 50 percent of the cost; or which have a deductible that only allows for “de minimis” reimbursement of services, or that the insurer knows will not be met by at least 80 percent of customers.
