Require unambiguous health insurance claims procedures

To require health insurance plans to establish clear and unambiguous policies and procedures for the submission of claims by health care providers and facilities, and prohibit insurers from changing or eliminating any coding, policy or procedure for the submission of claims, or their reimbursement rate or methodology, unless they follow procedures and timetables specified in the bill.
