Require certain dental insurance disclosures

To require health insurance or HMO policies that cover dental services to state in the policy language that the amount to be paid on a claim is the maximum amount the insurer or HMO is obligated for, and that the amount is not a determination as to the reasonableness of the amount billed or whether that amount is the reasonable and customary fee for the specific service.
