To prohibit a primary care physician from seeing a patient over age 16 unless the patient has an up-to-date government-issued photo identification card. Also to require an individual who has been convicted of a drug crime involving a prescription drug to disclose this to the physician. Then, on any subsequent visits, to prohibit the doctor from providing care unless the person provides a copy of his or her criminal record, which would be placed in the person’s medical records.
