Revise “durable power of attorney” details

To require a “durable power of attorney” document to be signed by the principal, dated, and signed by two individuals who witness the principal signing it. Among other things durable power of attorney gives a designated person the authority to discontinue the life supports keeping a terminally ill person alive.
