To increase the penalties for a health care provider intentionally, willfully or recklessly placing misleading or inaccurate information in a patient's medical record or chart. Also, to apply these penalties to a provider intentionally or willfully failing to place information in a patient's records for the purpose of concealing the providers responsibility for the patient's injury, sickness, or death.
